It's from the April 12th 1993 edition of Who Weekly, and yes the photo is completely uncensored in the issue.
It was part of a special fine-art photoshoot for the magazine featuring celebs (Naomi Watts, Deborah Hutton, Julian McMahon, Hugo Weaving, etc.) and their siblings posing nude (or implied) in a arty style. Although only Zoe, Gia and Deborah McMahon (Julian's younger sister, credited as Debbie McMahon) bare their breasts in the shoot.
